Watch: A Chiropractor Who Built a Category of One Around DOT Physicals

In this Drive-By SEO short, Joshua Albanese, founder of Spearleaf, looks at Doc On The Spot, a chiropractor that built a whole business around DOT physicals for truck drivers, as an example of his category-of-one idea. It is listed second in the local area on Perplexity, and he explains how website optimization and recommendations from other sites, the core of AI search optimization, send the authority signals AI chats look for.

So today we search for a chiropractor. And if you have a business that's a chiropractor, this is going to be a really interesting one. So we came across something I've never seen before, but it's called Doc On The Spot. This is one of my theories, which is called the category of one.

He essentially created an opportunity that really doesn't exist in a lot of other markets. I've never actually seen it. And what he created was the DOT physical. That's the Department of Transportation. And he created a whole business around it. In fact, when you look at it, he's got only trucks on his Google Business Profile, and it's really saying a lot about it.

He has 222 reviews, it's listed as a chiropractor, and he helps these truck drivers get the physicals necessary. This is amazing because he essentially established himself as an authority. And then, of course, he still has his other form of business, Doc Johnson DC. So he takes on his normal clients, and then he has this category of one, where he's really established himself to be the number one.

And when I go over to Perplexity and I take a look at it, he's listed number two in the local area. So Fast CDL is still listed as number one, but he's listed as number two, and I think the only thing that's really going to be missing here is just building more authority signals.

So if he was my client, how I would do this is I would look at his page. I'd definitely optimize his website to make sure that we were sending the right authority signals. And that means using the keyword research, but also I would find other websites, maybe even government websites, that we could get recommended by, and that will send the authority signals back to these AI chats, and then they're basically going to recommend him. So that way any truck driver that's driving down the road, they're going to be stopping by, and he's going to be making a whole lot more money.
